Do not use this application form for the following musical uses or products:
1. Advertising or similar commercial uses of music
2. Multimedia products: DVD, CD-ROM, computer and video games, etc.
3. Musical Merchandise: Toys, music boxes, greeting cards, or other novelty items
4. Internet and web site uses.


If you wish to apply for a license for the use of a musical work that falls into the above four categories,
please contact CMRRA's Synchronization Licensing Department to obtain the proper application forms
and documentation.


Please note:
1. A synchronization license grants authority to reproduce a musical composition only. It does not grant authority to reproduce a master recording. You must obtain a license for any master recording which is used in your production from the owner of such recording.
2. CMRRA does not accept applications for "gratis" or free synchronization licenses. You may offer, where indicated below, a proposed license fee, which will be considered by the publisher.
